How to Access Your Medical Records at Weiser Memorial Hospital & Clinics
If you would like to request copies of your medical records and/or a copy of your radiology images, please fill out this authorization form.

You can return the form by:
- Mailing it to the address on the form
- Faxing it to 208-600-0817 *must have a hand signed signature and include a copy of your ID
- Emailing it to medicalrecords@weiserhospital.org *must have a hand signed signature and a copy of your ID
Please ensure the form is completely filled out and signed. Note – incomplete forms cannot be processed.
If you would like assistance in completing the form, you can visit our offices at the WMH Office Complex at 650 State St in Weiser or call us at 208-549-4455.
Once we receive your completed request, medical records will be mailed after 1-2 business days.
