The Daisy Award Recognition Program honors and celebrates the skillful and compassionate care that nurses provide to their patients every day. 
  At Weiser Memorial Hospital extraordinary care, of each and every patient, is a job that our nurses are passionate about.
  The Daisy Award is a recognition program started in memory of J. Patrick Barnes. Pat’s family experienced first-hand the difference his nurses made in his care through clinical excellence and outstanding compassionate care. The family created the Daisy Award to express gratitude to nurses and to enable other patients, families, and staff to thank and honor their special nurses.

How to Nominate an Extraordinary Nurse

Patients, visitors, nurses, physicians, employees, and families may nominate a deserving nurse by completing this form. As a deserving recipient of the Daisy Award, this nurse’s clinical skills and especially his/her compassionate care exemplifies the kind of nurse that our patients, their families, and our staff recognize as an outstanding role model. She/he has consistently me the following criteria:
