  • Andrea Wheeler, NP

    Family Medicine


    Andrea Wheeler is a Nurse Practitioner with 26 years of nursing experience. Nursing has been more than a career for her; it has been her calling. She spent much of her career in ICU and critical care, caring for patients and families during some of their most difficult and vulnerable moments. For the past eight years, she has served as a Nurse Practitioner, and over the last five years, she has found a deep love for women's health and hormone therapy as a Certified Menopause Provider. She earned her Master of Science in Nursing from Walden University after completing her undergraduate studies at Boise State University. Her heart for nursing comes from a desire to help others feel heard and cared for, while walking alongside them in their health journey with compassion and understanding. 


    Outside of her career, her greatest blessing is her family. She is a proud wife and mother of five wonderful children. They love spending their free time together outside exploring, at home relaxing or simply making memories as a family. She enjoys reading, preparing healthy meals, baking, and creating a warm, welcoming home. Being a wife and mom is one of the most rewarding parts of her life. Whether at home or in her work, her heart has always been centered on loving people well, helping them feel valued, supported, and truly seen.

  • Jon Hlavinka, MD

    Family Medicine


    Dr Jon Hlavinka followed in his father’s footsteps as a “hometown family doctor” and has been practicing medicine for 40 years. He has seen it all as medical care has evolved.


    From his hometown of 1800 in rural North Dakota to the wilds of Alaska where he served for 6 years as a “bush doctor” and director of Kodiak Area Native Association.


    After this experience he worked at Saltzer Medical for 30 years and was one of the founding physicians when they were Med Center One with Mercy Medical in Nampa, ID.


    He has also worked in obstetrics, as a nursing home director, a sports team doctor, and oversaw refugee health care at Boise Mental Health Center.


    Dr Hlavinka trained at the University of North Dakota School of Medicine and completed residency training at U of W Spokane.


    Dr Hlavinka started his family and moved to Nampa in 1992. Both of his children played fiddle and were involved with the National Old Time Fiddle Contest in Weiser. Over the years he has biked a fair amount of the rails to trails in Weiser. The community of Weiser makes him feel at home because it is rural and agricultural - just like his hometown. Dr Hlavinka has coached soccer and has been involved in road cycling with his children.


    His passions are his “hobby farm” with chickens, goats, gardens, flowers, and fruit trees with his girlfriend. He also enjoys photography and has always had a passion for fitness. Dr Hlavinka is an outdoor enthusiast over the years has settled into paddle boarding, snowshoeing and scuba diving. He loves to travel, especially internationally.

MEET OUR BEHAVIORAL HEALTH TEAM


We are an integrated behavioral health center which includes a psychiatric care team along with our medical care team. We provide services for all ages and coordinate medical and behavioral health care among our providers. We work as partners with our patients to determine treatment plans and bridge gaps for services needed.

  • Stephanie Gallegos, LCSW BHC

    Behavioral Health Consultant


    Stephanie Gallegos is a Licensed Clinical Social Worker (LCSW) and Certified Perinatal Mental Health Professional (PMH-C) dedicated to providing compassionate, evidence-based, behavioral health care within primary care settings. She earned her Master of Social Work degree from Northwest Nazarene University and has extensive experience helping individuals navigate the connection between physical health, emotional well-being and behavior change. Stephanie works closely with patients and medical providers to address concerns such as chronic disease management, anxiety, depression, stress, life transitions, and health-related behavior changes. She is also considered a Tobacco Treatment Specialist and has specialized training in Cognitive Behavioral Therapy for Insomnia (CBT-I). Her approach is compassionate, collaborative, and evidence-based, with a focus on helping patients develop practical skills to improve their overall health and quality of life. Stephanie is passionate about increasing access to behavioral health services and supporting whole-person care within the healthcare setting.


    Outside of her professional role, Stephanie's greatest joy is spending time with her husband and twin sons, who keep life active, adventurous, and full of laughter. She cherishes the everyday moments of family life and values creating meaningful memories together, whether exploring the outdoors or enjoying time with loved ones. As both a behavioral health clinician and mother, Stephanie understands the challenges of balancing competing responsibilities and the importance of caring for one's own well-being. These experiences help shape the empathy, authenticity, and practical perspective she brings to her work, helping others navigate life's challenges with confidence and resilience.
